Lilbourn is located at 36°35′27″N 89°36′46″W 36.590859, -89.612762. The city is concentrated along County Road D 3rd Street, just north of the roads intersection with U.S. Route 62. Lilbourn is flanked by the small village of North Lilbourn to the northwest. New Madrid lies to the east, and Marston lies to the south. The Little River passes just west of Lilbourn.

According to the Bureau of US census, the town has a total area of 0.94 square miles of 2.43 km2, all land.
